Sticking rpms?

My car is in good shape now, got a new alternator, new oil, filters and plugs, but I still have an issue with idling and sometimes starting. Seems like something is sticking or something and the idle seems to vary depending on situation. I'll describe it below and would appreciate some ideas on what I could check to remedy it:

  • The car always starts perfectly first time from cold.
  • From warm or hot, it usually starts fine, but sometimes cranks for a bit before firing up and sometimes when it does start, struggles for a few seconds and then straightens out.
  • When in DRIVE (yes it's an auto) and I have my foot on the brake, the engine seems to pulsate, like a slow vroom, vroom, vroom, as if it might cut out, then it comes back.
  • In neutral, it idles faster
Someone mentioned before the EGR valve being clogged? Advice on how I can clean this issue up appreciated!
 
Can anyone offer some advice? The other day I started car up and seemed to run up to high rpms for a second and then drop back to idle.
